The complainent needs to submit the following affidavit on Rs 20 (or more) stamp paper with the text mentioned below, as per instructions of the Government of Pakistan Contained in letter No 8/372/80-Imp dated 26.06.1980

I ___________________ S/D/W/O___________________ solemnly do hereby declare under oath that the contents of the complaint dated___________________, filed by me, are based on fact and nothing has been concealed misstated therein.

Please send hard copy, affidavit of application alongwith CNIC and supporting documents at the address mentioned below after filing complaint online.

Introduction

Citizens can use the following means to file a complaint against corruption of FBR Officials:

  1. Citizens can use FBR hotline 111-772-772 to file a complaint.
  2. Citizens can use FBR website www.fbr.gov.pk to file a complaint.
  3. Citizens can send an email at complaints@fbr.gov.pk to file a complaint.
  4. Citizens can lodge complaint via post to "Second Secretary (IMC), Room 505, Constitution Avenue, FBR house , Islamabad".
  5. Citizens can lodge complaint via meeting in person with "Second Secretary (IMC), Room 505,Constitution Avenue, FBR house , Islamabad".

How to File Corruption Complaint Online

  1. This form can only be used for filing corruption related complaints against officers/officials of FBR
  2. Once clicked a form will appear which needs to be filled out.
  3. Following fields are mandatory for filing complaints.
    1. Name of the Employee
    2. CNIC
    3. EMAIL
    4. Cell Number
    5. Postal Address
    6. Affidavit
    7. Designation of the Employee
    8. BPS
    9. Description of the Complaint
  4. After providing all the information above you need to submit an affidavit on RS 20 or more stamp paper as per the instruction of the government of Pakistan contained in letter NO 8/372/80-imp dated 26.06.1980.
  5. Please attach the affidavit as mentioned in point-4 along with CNIC and supporting documents and submit complaint.
  6. The system will generate complaint ID assigned to your complaint for tracking progress.
  7. This complaint number can be used online to track progress of your complaint
  8. Please send hard copy of complaint, affidavit, CNIC and supporting documents at the following address

How to File Corruption Complaint on Hotline

  1. Citizens can directly call at hotline 111-772-772 to file a complaint.

How to File Corruption Complaint via email

  1. Following fields are mandatory for filling complaints via Email.
    1. Name of the Employee
    2. CNIC
    3. EMAIL
    4. Cell Number
    5. Postal Address
    6. Affidavit

    9. Description of the Complaint
  2. After providing all the information above you need to submit an affidavit on RS 20 or more stamp paper as per the instruction of the government of Pakistan contained in letter NO 8/372/80-imp dated 26.06.1980.
  3. Please attach affidavit as mentioned in point-2 along with CNIC and supporting documents while sending email at complaints@fbr.gov.pk
  4. A complaint number will be sent to you through email right after you submit complaint online
  5. This complaint number can be used online to track progress of your complaint.
  6. Please send hard copy of complaint, affidavit, CNIC and supporting documents at the following address
